A broad service catchment

CCC is the designated preferred provider of cardiac outpatient prevention and rehabilitation services for Cambridge Memorial Hospital and the Waterloo-Wellington region. We are also a founding affiliate member of the Cambridge North Dumfries Health Team. As such, Cambridge Cardiac Care is a part of a broadened circle of care, serving patients both from the immediate communities of Cambridge, Kitchener-Waterloo and Guelph as well as patients from all across southwestern Ontario. Our virtual e-visit option for patients and e-consult option for healthcare providers allow us to serve even at a distance.

Our adult and pediatric cardiologists are conveniently available for eConsult to physicians and nurse practitioners across Ontario, via the OTN eConsult platform. This secure, web-based tool efficiently connects you to the specific specialist you need within 24-48 hours, improving care coordination and sometimes even eliminating the need for patient referral altogether.

Cambridge Cardiac Care strives to provide timely care for your patients, regardless of their location or mobility restrictions. As such, we are pleased to offer virtual cardiology services to out-patient referrals including specialized Rapid Access clinics, Cardiac Rehab & Risk-reversal programs as well as direct cardiologist consults. Virtual appointments may be conducted by video or telephone based on patient preference.
